Error Fetching DataSource

Hi All, 

I am trying to create a Form Data Model, but it is not fetching the DataSource I created. I uploaded and installed the current JDBC Driver for MySQL. I then created my DataSource, and I will have a screenshot of that below. I have already tried this configuration in my local instance, but when I decided to upload AEM Forms and create a FDM in our DEV instance, I am getting an error.
