Custom header in pdf using fmdita(XML Documentation Add-on)

AEM - 6.5
com.adobe.fmdita-6.5-3.6.170-add-on

 

We have two requirements
1. Need to have company name, logo and caption in pdf first page alone.
2. Need to have company name, logo and caption in header in all the pdf pages.


Do we need to have achieve this using custom pdf plugin? And which files needs to be changes to achieve these.
