AEM 6.0 SP3 won't install

Hello, 

In my development environment, I have 3 servers: one author and 2 publishers. All 3 previously had 6.0 SP2. I upgraded to SP3 on all 3 environments. Only the two publishers actually show the correct oak version (1.0.22) and "SP3" displayed on the welcome screen. The author server, for some reason, will not take the upgrade. It's still stuck on oak 1.0.18 and won't take the upgrade. I have tried reinstalling the package a couple times now with no luck. 

Any ideas where to start with this?

From the log I see the install started at 

20.11.2015 13:33:55.237 *WARN* [qtp450911036-34] org.apache.jackrabbit.vault.packaging.impl.JcrPackageImpl Refusing to recreate snapshot adobe/cq600/servicepack/.snapshot:aem-service-pkg:6.0.SP3, already exists.

The last message is at:

20.11.2015 13:35:21.094 *INFO* [qtp450911036-34] org.apache.jackrabbit.vault.packaging.impl.ZipVaultPackage Extracting day/cq60/product:cq-ui-classic-content:1.0.190 completed.

Usually I would expect to see a lot more messages when a SP is installed.

To deal with this situation you can try to uninstall the package, delete it from the repository and try again. Or raise a daycare ticket to debug the issue. Or recreate the instance from a copy of the your other publish instance and debug at a later time.

Are you seeing errors in the log file?

When you install the package from package manager, does the install complete without error. In the error.log file, the install process will keep on running although the package manager says it is completed, does the install complete without error in the error.log file?

The package in crx package manager shows that it was successfully installed so no issues there.

I am seeing one error message in error.log over and over but not sure it's related. See below. I also attached the error.log from today when I just tried to install again with no luck.
