using Form block in Edge delivery service is not generating form (Document based authoring)

Hi, I wanted to learn about form block on Edge delivery service using document based authoring and came across one problem in which I am not able to render the form even after configuring and following all the steps .

I followed the tutorial : https://experienceleague.adobe.com/en/docs/experience-manager-cloud-service/content/edge-delivery/build-forms/getting-started-edge-delivery-services-forms/tutorial

 

and created the page https://main--eds-forms-poc--rahul234dabas.hlx.page/newenquiry  using the google doc : https://docs.google.com/document/d/1i3dezDbfuBYuWPeG7M_yoigKZCuts2SAGBU1QCqIgFI/edit?usp=sharing

I have used the GitHub template provided by Adobe: https://github.com/adobe-rnd/aem-boilerplate-forms.

I have followed the necessary steps but instead of showing the form , only link is being seen on the page, please tell me what steps I am missing and what needs to be done?

Your code is Outdated, please take latest code from repo.

I can see in your code form and submit links are mandatory.
