Text elements in a Form

What's the best way to extract ALL the Text elements (Text strings in Text objects, Captions of Fields...etc.) in a Form?

I'd like to put all of them in one txt or word file.  Just want the strings, don't care about their formatting or position.

Did you try opening the form in Adobe Reader/ Acrobat and choose Edit menu -> Select all. This will select all the text in the PDF.
