A "Save As" button that inserts values from fields in a form

I know I have seen this in the discussion threads but can no longer find it.  I want to add on to my "Save As" button to have it choose two field values to automatically generate a name for the file to be saved.  Specifically using the name and date a person enters on the form.

You will have to use a folder level script for this purpose.
