Setting up Request Default Approval

I need help in figuring out how to correctly implement a Default Approval team for an issue using a queue topic. I have one correctly set up (EBCG attachments below), but the other (FI attachments below) won't work correctly, despite the fact that all seems the same.

This has been resolved; I didn't have an APPROVAL PROCESS on the issue.
