Is there a way to add the Due Date and Assignments to the Issue Form?

Is there a way to add the Due Date and Assignments to the Issue Form? I need to be able to have the person who initially enters the issue to be able to assign the issue right then and there and put a due date on it so that it immediately shows up for the users to log time to the issue. Is there a way to add Workfront fields to a custom form?

In Queue setup, you can add the Assigned To field and the Planned Completion Date so they are visible when adding an issue. I don't know how you'd be able to add a Workfront field to a custom form.
