...is it possible to create a Task report that also shows me the Queue Topic that was chosen at the time of logging the original request? And if I could also be cheeky and throw another one in: in a Task report, how can I show the link to a document that was attached to that approval task? (I'm filtering on "Is Approval" = Yes)

Instead of trying this in a task report, you can create an issue report and make it look like a task report. The format would be something like this: Issue ID Queue Topic Name Resolving Task Name Resolving Task Other Stuff The latter is a little too cheeky for me.
