In similar fashion to the Issue and Risks tabs, the introduction of a Action Register is of value to identify decision points, external deliverables, and requests for information, to name a few.
Not a new Idea to this exchange, i would argue that Issues have their specific purpose... that being something has gone afoul with a task. Additionally, the term Issue itself carries a negative connotation. While Terminology can be used to change the name of Issues, implementation is very unwieldy.
