I'm new at this, and although your answer was helpful, I still don't understand how to make the Subject of the email populate from the "Name" field in the attached fillable form.I would also like to rename the form as well, as the "Name" field.  Any help you can provide, would be greatly appreciated...

Mine flows to the 2nd page OK, but then the it overlaps my header that I've built on the Master page.  Any ideas on how I can avoid that?
