I have simple spreadsheet data that includes zip codes and their corresponding city/town names. I want to add a table to a livecycle form (Pdf) so when I type in the zip code field, the city/ town field is auto-populated. Example:27105 = Winston SalemI want to type the zip 27105 and have Winston Salem appear in the city/ town field.

I created a fill-able PDF. The user then hits a "button" which executes the send via email. When the information is transmitted the form comes back blank even though the use has completed it. Is my form corrupt or is it set up incorrectly?Thank you.
