Adobe Form Designer is not sold separately anymore, it's provided as part of a server license of AEM Forms.To answer your questions, the form must be Adobe Reader Extended in order for the dynamic barcode to work with Adobe Reader. It's not there for Adaptive Forms at the moment and something we're looking to add to a future release based on demand to drive the priority. Thanks for indicating the interest and I've let the Product Management team and engineering know there was a request for this capability.

For fields we have a concept of widgets (java script). Since Adaptive form fields have a lot more capabilities, such as, initialization/calculation/enabling/visibility rules, etc. A simple static HTML component does not suffice. You can create a PDF from a form submission and there is no requirement to store the data or PDF in the CRX repository. You would need to implement the integration into your back end. You would have a single template that has the header and footer information. When a form is created the header and footer on the template would appear. The downside is if you change the header and/or footer then the template would have to be applied to each of the form designs to reflect the update. You are trying to do this using AEM Forms JEE and not an AEM Workflow correct? If yes then I've attached an LCA that you can import through adminui that is working on my AEM Forms 6.2 JEE install. Rename the file from .txt to .lca to do the import

Yes you can create two fragments and you'll notice there is a header and footer component in adaptive forms. If the header/footer information changed you could edit the fragments and any existing (in progress forms) or new forms would show the updated information. If you changed a template you have to go back and update the existing form designs with the new template.

If the header and footer are the same why not create a template with the header and footer or create form fragment header/footer elements. Using fragments allows you to modify them as needed and have the changes replicated automatically across forms.

If you're connecting to ES2 the credentials wouldn't be admin/admin. That's the default for AEM. For LiveCycle or AEM Forms JEE the default is administrator/password but if this is a production instance that may have been changed.

This is to completely correct. Data from a form is stored in a database. Documents such as attachments are typically stored on the file system.
