Upon opening in Acrobat, I see 2 'remove row' buttons

PROBLEM:

I have an 'Add Row' button and there should be only one 'remove row' button at the bottom of the table.

However, upon opening the LCD dynamic form in Acrobat, there are always 2 Remove Row buttons. I can click on one and then I have what I want.

BACKGROUND:

I apply the permissions for saving and so on in Acrobat after removing the extra button, but when the .pdf is opened later, there are the two buttons again.

One thing I found in 1 of the misbehaving forms is some traverse/traversal lines - I deleted them and now the form works. I looked in the remaining 'naughty' forms but there is no traverse/traversal lines to delete. I found no differences.

QUESTIONS:

Is there any reason this is happening?

Is there a way to fix it or to stop it from happening?
