Unusual Text in Form

I am transforming an existing PDF form into one that can be filled in and printed.  I have added check boxes and also text field.  When I save the form as a PDF, there are the words "Text field" just to the left of the text field.  How do I get rid of this?  The form already had text describing each field to complete.
