Render as html and include all pages.

I am new to Livecycle and I am currently developing a form. I am using Adobe LiveCycle Designer ES3. I want to render the form as html and be able to submit the form using a button that will go to a form server and be merged with the xdp and xml and produce a pdf. The html that I have come up with so far does not look well. I have a 2 page form but I a only getting one page. I am not sure what I am not including. I am rendering as  AccessibleHTML. Any help would be much appreciated. Thank you!

Is there a chance you could move to LiveCycle ES4?  There is a new rendering engine to create HTML5 and will provide a much better rendering of the xdp.  To test you can preview the HTML from within the Desinger (a server is required) or use the mobileformsivs test harness.
