Submit form to email addressee of the user's choice and submit again to mailbox

I am using Adobe LiveCycle Designer and am new to this and not a programmer (tech writer who designs forms). I need to have the user submit the completed pdf form to their manager and pick the manager's email address. I want to control the subject line. Then the manager needs to fill in two fields and submit the form to a mailbox that I will control.

I read a similar request below but need specific Java scripting help since I don't know that part.

Take a look at Paul's form. There are mail adresses set by choice.

You could also make your own choice by using a counter (invisible Decimalfield that will add +1 when the user for example opens the document and set the mail adress according to it).
