Thank you for taking a look at this.

I really need to get this project completed because it is part of a larger task.

The simple problem is that I created this form, which seems to work for the first ‘premises’.  However, when I click on ‘add premises’ at the bottom of the form, the next form does not work at all.  None of the script seems to work.  None of the fields are hidden.  None of the check-boxes work…. Etc.

I’ve attached a ‘non-password protected’ copy of the form for your perusal and encourage you call me at 727-498-0148 if you would like to ask any quesitons.

What exactly is not working?

When I press the button 'Add Premisses'  a new page is added.

When I answer the following question with 'Yes' an extra line is shown to ask for some details:

Does the premises have a Fire Alarm?

Hi Ronnie,

Property Underwriting Questionnaire V2.0_Page_1.tiff shows the form, prior to adding another premises.  Everything seems to be in order.  The feilds will dynamaically expand depending on selection.

Property Underwriting Questionnaire V2.0_Page_2.tiff shows the form, after clicking 'Add Premises'.  At this point, it should look and operated the same as the first page (Property Underwriting Questionnaire V2.0_Page_1.tiff); however, all the fields are visible.  When you check 'YES', 'NO' should Uncheck, as on the first page.... and so on.


This is very frustrating in that I can't seem to get anyone to see the problem.  Please feel free to call me if you have any quesitons. 727-498-0148

Can you submit your second tiff. I got 'qeued'.

Hi,

Found your problems, look at the scripts on the objects for the 'Fire Alarm Information'.

I made some changes and commented them.
