I created a fillable form and included the submit by email button. 

In testing, the email goes to the address indicated however it comes as just a sting of text of the field type until it gets to name and address and those I can read.  I can't figure out how to submit as something other than a .xml file.  Thank you.

Yes, but from your screenshot I see this is a LiveCycle Designer form. This forum is about Acrobat forms.

Called Adobe LiveCycle Designer, which is what came up when I was in Acrobat and said I wanted to create a new form...  Is there a LiveCycle Designer forum?

Yes, I moved this question to it: LiveCycle Designer

If you haven't done much work on it yet I would recommend you start over and create an Acrobat form. It's much more widely supported and more likely to work in non-Adobe applications, as well as mobile devices. The two formats are not compatible with one another, though, so I'll have to start from scratch, pretty much.
