How to create a time and date stamp to act as an esign

I am creating a form to be posted to our website for patients to fill out on line and submit.  I need a signature, but believe I the individual person filling out the form would have to create their own electronic signature.  To get around this, I thought I could use a time and date stamp that they could click after reading each statement, but I do not need this button to also submit the document.  That will come at the end and I have that in place already.

Can anyone help me in creating the time/date stamp button?

Are you creating the form from an existing document using PDF Generator?  (For example converting a Word document to a PDF server sided).

If not, and you are creating the document using LiveCycle Designer, try publishing this question to either the Forms or the Designer forums.   You'll likely get a better response to your question in those forums.
