Add free text entry box

I need to add a free text entry box after each radio option. For example, Question: Which fruit is your favorite, and please specify the reason.

A. Apples, because ____________________________________

B. Cherries, because ___________________________________

C. Bananas, because___________________________________

D. Blueberries, because_________________________________

E. None of the above, please specify ______________________

How to add these kind of questions using Adobe Livecyle Designer?

This is pretty easy depending on what you want to do. If you just want to align a field to the right of the radio button then you can just align them. However if you want the radio button when selected to then display a text field for the user to enter more data, you will need to script it. If you explain with more detail I should be able to create a simple sample

You would have to essentially create the radio buttons with the associated fields. The fields will be hidden and upon click they will display. I can create a sample if that will help. Send me our email and I can get it out to you

Your solution is exactly what I need. I'm also OK if the fields are already displayed even the corresponding radio buttons are not selected, which would not look nice but it could still do the trick.
