I am receiving a large number of references from suppliers about a variety of services. I want to pull the name of the service, the company name, and the reference name (The individual person I'm surveying) from a spreadsheet dynamically. I want the form to be sent to the corresonding e-mail address. I only need to get a simply 1-5 rating for the service to come back to me. I then need to be able to have the 1-5 rating along with all the outgoing info to come back.
Can it be done?
Can you tell me what functionality I need to use?
I just need to know where to look to learn how to do it, but first want to know if it is possible.
There will be hundreds of individual services. Each service will have three references. Obviously, I don't want to type it all in. I see it very much like a mailmerge, but in Acrobat simply don't know where to start. I have been building static survey forms for a year. Moving to this is a bit daunting. If you know of an article or book that addresses this in a straightforward manner, I'd be grateful for any help.
