Livecycle Form Header & Duplication Issue

I am creating a form in Livecycle Designer ES version 8.2 that is 7 pages long and it is in a large table with a header and allows page breaks. The header appears on the first two pages, but not any afterwards, has anyone had this problem? I checked and a new table is not created, there is not break, it just does not show up. Also, this form will not allow row duplication, any thoughts? This has me stumped and I have not found anything online regarding an issue like this.

I would need to see the form in action to detrmine what is happening ...can you send it to LiveCycle8@gmail.com and I will have a look when I get a chance. Please include a description of the issues with the form when you send the email.
