LIVECYCLE Form Distribution and Data Collection

HELP!!
I have created a PDF form using Livecycle Design and I intend to distribute the form to various users; and collect the responses back with the in-built feature in adobe pro 9 ( there is this file called RESPONSE FILE (when I distribute the form) where I can view or collate all the response forms submit to me)

My issues is as such that;-

1. I have ONE form
2. There will be 3 main centers i.e. Centre 1,2,3 which the users can send the forms to
3. How can I design the form in a way that if User A chose to send the form to Centre 2; Centre 1 and 3 will not get the distributed form sent to them?

I hope the question make sense and it will be of great help if sombody provide me with some guidance!!

Hi,

You can set up a custom submit button that will use a different email address at runtime, depending on users input (eg which Centre they select). There is a sample here (you want to look at the script in the click event of the first email button: https://acrobat.com/#d=JXrO*Eb8bm9jtqFWwxYmsA

However, if you are using Acrobat.com to distribute the form and collate the responses, then the email address attached with that Acrobat.com account will override the email address in the submit button. You should be able to test this before you deploy your form.

One workaround would be to distribute the form outside of Acrobat.com and then manually collect the responses in each centre.

this seems counter productive requiring the use of the subscription email address as the most obvious link for the user
