Issue with renderFormGuide activity in Live Cycle ES2 (version 9)

Dear All ,

In Adobe LiveCycle ES2 (version 9), we no longer create form guides with Adobe Designer ES2 version 9.

I am not able to render a form guide using renderFormGuide activity  in workbench , The issue , I am facing is partianing to url options

section . I am not able to figure it out what will be the  Target URL , Content Root URI , Base URL

and Form To Render . I tried different options  mentioned in help docs

There is  no sample avaiable in LiveCycle ES2 (version 9) which shows the use of renderFormGuide activity

It will a great help , if  any body can provide a sample or atleast direct me to some links .......
