Is there a way to create a form that shows a specific header based on the item selected in the drop down menu?

I have created the form and bound the data to the specific product name however I can only get the header for one item to show up and cannot get the header that is specific to the item selected in the drop down menu to show up. I hope I explained this correctly. I have been working on this for weeks.

I think we might need to see the form to understand the problem.  Can you upload it somewhere and post a link to it in this thread.  Maybe some pictures would be enough.
