how to import an excel spreadsheet with formulas and then submit the form?

I've got an order form that I'd like to convert to an interactive PDF that people can fill out then submit to the client.  (I've done this before albeit only with resume data on an application.)

How can I grab the spreadsheet complete with the sum formulas and bring them into Live Cycle?

...basically: quantity - amounts - totals per line with a grand total at the bottom....  using the SUM and Multiplication to Calculate totals with Gratuity and Tax.

I've attached an image of the spreadsheet...
