How can I get my LiveCycle PDF form to incorporate text box info to Excel?

I've created a questionnaire in Live Cycle ES4 and am unsure how to get the information that's filled in the text boxes from that file to automatically populate in an excel spreadsheet.

Is this possible?  If so, please advise.  Thanks!
