Help needed with feedback box (tick 1-9)

Hi Community,

I want to create a feedback form that uses rating boxes from 1 to 9 to give feedback for questions.

The image below shows the current look in Word.

Now I´ve got the problem that I don´t seem to get how to create such a box in Acrobat X Pro or Acrobat Livecycle Designer and there is absolutely no tutorial on the net.

I guess there must be a convenient way due to to Acrobat being perfect for forms.

Does anybody have a hint?

Hi,

In either Acrobat or LC Designer you can drag on radio buttons, which when grouped will be mutually exclusive. I would recommend that you stick with the default circle shape for the radio buttons, as the user will know that they can only select one from the selection.
