Form not printing correctly with HP OfficeJet 4215xi

I have a fillable form created in Adobe LiveCycle Designer ES 8.2 that was printing fine on several printers. Suddenly stopped working on this one printer - the HP OfficeJet 4215xi.

This is on Windows XP Home Edition.

Tried updating the printer driver, tried uninstalling Adobe Reader and reverting to Adobe Reader 9.5. Nothing seems to help.

Anyone else experiencing this problem or similar? Any suggestions? This is the setup we need to have this form working on!!

To clarify - it's not printing the captions for the fields, just the boxes.

try this common workaround.

In Reader goto File > Print > Advanced and activate the check box "Print as Image".
