Hi! I have made a number of forms with floating fields before- they all fill in just fine. I can't figure out what I'm doing wrong with this one, though.

The above is a form with the fields working. I have looked at the settings and tried to compare with the form above, and as I said, I just can't see what I'm missing.
Any ideas?
