Error Message with FormCalc

Hello,

I am a beginner with Form Calculations and am trying to average the score (Total Traits Score) (from a set of four radio fields (with five buttoms each (1-5))) with another input (Cultural Fit) - Also a radio field with buttons 1-5).  I have been able to figure out how to set up a numberical field to calculate the average of the first four radio fields (the Total traits Score), but now I need to average that with the fifth radio field (Cultural Fit) and I am receiving the following error message.     Any advice on why this is happening and how to fix it?

Here you have to give proper path reference for the field "Total Traits Score". Here in the script it's unable to find the field.

Thanks Bibhu but I have checked the path reference several times and do not understand why it is unable to find the field?  Is there an issue because I am asking it to find a numerical field that is only populated after a selection is made for the radio blocks above?
