I'm using forms to gather responses to documents we post for comment. Our website renames all files with a numeric name, so it's hard to distinguish between the files when they are received using the default subject line. I would like to set the subject to something like "Response Form: subject", but can't find a way.
When I edit the object settings for the button, I can type in a subject, and it works if I click 'submit' in Acrobat. However, after distributing the form, the subject does not carry through the distribution process when I click 'submit' in the new, distributed, file. I need a reasonably simple form that will work with submitters of various skill and various acrobat/reader versions. I have attached an example form.
