Drop Down List

I have a drop down list in my form that I want to show and hide fields in my form. The drop down list consists of:

-Customer Meeting

-Customer Video

-Customer Notes

What I want it to do is have it where you choose one of these from the drop down list, and only the fields associated with that section shows and the others are hidden. Can someone help me out?

I was looking for that to build a generic fax template for multiple locations.
