After permitting LiveCycle to track my use of the software, I have not been able to load my document since.  I've been trying now for about 5 hours and the software just doesn't load.

I get a C++ Runtime error about every 7th or 8th time I attempt to save my document after editing.  What's worse than that, it dumps my document.  I have learned to have two documents and back-up each so that I don't lose so much data.

I am creating a medical diagnostic form and need to put in the client's name, record number, etc at the top of the form so that it automatically prints on all 20 pages.  How is this done?
