Adobe LiveCycle (Archived)

I have a document that I created in LiveCycle Designer.  I have an order form that is attached to the pdf when I bring it up in adobe to complete the form.  The attached form has check boxes that can be marked to complete the order form, but I want the text on what the check box was for to translate...

I am trying to figure out how to take the answers from check boxes on one form that is attached to another form and make them type into the second form. For example, if the check box says right foot, I want it to say right foot in the first document.  How do I do this?
