Attachement Problem In the workspace

In my process i want to attach certain file sin my pdf form.i have reader extended the forms and when it is coming to start process,initial user can add the attachement.But when it comes to next user he cannot view that file along with the form .What could be the problem?

First condition: You process's input variable must be of Document type (not XML variable)

Second Condition: You save the whole PDF instead of the form's data in the document variable.

Thrid Condition: The next actor's Presentation & Data property must be set as 'use a document variable'

Make sure that all the above conditions are true to your design.
