Assign Task - Something looks weired !!!

My process contains a AssignTask activity. Sometime when a task is assigned to a user it creates duplicate tasks (i.e two task instances to the same user)

See the attached screenshot taken from AdminUI. What could be the reason?

We have implemented a cluster environment with two nodes. Is this causing this issue?

Here is another task which gets duplicated. Someone please help me soon.....

After removing one of the node from the cluster, it works fine.

I don't know why this is not synchronized properly.

Anyway, this let us to stay away from cluster head-aches..
