addInstance not working

I have added a button to a form that should add a row. I have been able to get and add item type button to work in other forms, but I can't get this one to work. Can someone take a look and let me know what I'm missing?

Thanks,

MDawn

The form is staic ...save it as a dynamic form and it will work fine.
