someone please help me with excel

I have created a form for our supervisors to fill out reviews on the employees. the top of the form has the employee info like name/ employee #, position, etc. In the past the form was a word document and we merged it with an excel database. that way if we had 10 employee reviews to do, we would pull the information from excel and it would fill in the fields for each employee. Can someone help me do this with designer 7.1 I can't figure this out. I was told it could be done in 7.0 then told 7.1. I have an interactive PDF form that has about 15 fields that I would like to populate automatically from information on an excel spreatsheet.
