Pdf form

I need to make 2 pdf forms. There are not too complicated, only some images and some text fields.
I have searched the internet for a company or some person who does this but I did not find anyone.

I have studied a little and I don't now if it is so easy as I thought.

I have attached 2 files with what I intend to do. Can some experienced user, please tell me if it is possible to do that?
