Form with field for attachment

I need to create a form that will allow the user to add an attachment and then email both the form and attachment. The users are from outside the company and are unknown. I'm running LiveCycle Designer 8. I don't see this capability with the library objects available. All constructive help is appreciated.

Hey Can you send me that as well. I would appreciate it. I would also like to have the submit button linked to a email addresses that are filled out in the form. example by pressing submit it sends to email address 1, email address 2 & email address 3.
