Filled out PDF form, will not print even when printing is allowed???

I created a simple, interactive form from an EPS file and manually placed all text fields. When the form is opened and filled out in Acrobat Reader, it will only print the original form, leaving all text fields completed by the user blank. I looks like by default the form has not printing restrictiona and even says in the PDF security that printing is allowed. I also tried setting up a password in the Permissions area to restrict printing to low or high resolution and still no printing.

Does anyone know how to allow the filled out form to print?

Thanks!

The I tried it in Reader 6.0 and the latest downloadable version from Adobe's website - 8.0.

The form was created in the Life Cylce program that comes with Acrobat Prof. in CS3.

Thanks so much for your time.

PS I can email the form if it makes it easier.
