Empty Survey When New User Apply

I have created a satisfaction survey, and have uploaded it to my website.

Whenever people click on a button, my document shows up in their browser.

Now they fill out the form and submit when finished to send the result to my email address. Everything works out fine till now, but here is the problem:

Whenever the document opens again the result from the former user is still there... All the things he/she typed in the form are kept, and the form does not "reset" it self.

How can I make my survey work so it´s just a blank form when it is opened up???

I don't know the reason this happens..(it doesnt happen to me!) maybe there's a simple solution - but since nobodys giving you a solution, i think one solution would be to add a script to the 'layout ready' event of the form that would loop through all fields in each subform & set each field to its default value.

One has to be careful of the caching of data and you may want to reset the form upon opening.
