Have you checked out Adobe's new cloud service - FormsCentral?

It is an easy to use, drag and drop for fill and submit forms. It seems to make it very, very easy to amend the built-in templates, deploy the form to your audience and then analysis the responses.

As far as I can see, it currently does not allows scripting. Since it is not pitched as an advanced development tool requiring programming skills, I doubt if it will ever have scripting ability. So things like calculations, dynamic changes are out. However for a "fill and submit" type form it certainly fills a gap.

I tested it yesterday.

It's simple to use and has enough features for basic surveys or such things.

Very end-user-friendly, but currently only available in english.

Hopefully it will be extended soon.

One positive, is that the online forms can also be accessed, completed, and submitted from an iPhone. I wasn't expecting that given the Flash issues; however total inclusion - result!

The aspect it is accessible from mobile devices is really a benefit of FormsCentral.

If they will provide an api this can be very popular I think.
