problem in creating simple dialog in AEM and add dropdown in dialog

Hi guys,

I am following this link http://www.aemcq5tutorials.com/tutorials/create-dialogs-aem/#collapsible_field_aem  creating simple dialog  but when i add widgets in dialog one is text field and other is dropdown list and i click on component ,dialog is popup but blank dialog is popup my textfield and drop down selection list is not appear

All the steps that is mention in the link followed successfully .

I would suggest to use Touch UI Granite dialog instead of legacy ExtJs xtype
