Error in fetching data source in a form

i have added jar files in bundles and also i have made changes in 

Apache Sling Connection Pooled DataSource

but still i am not connecting the database AEM 6.4

where i maid mistake ? please tell me

Hi, do you have any more information on the error? What does the error.log file say?
