Could not start a workflow

Hi All,
We are facing an issue in workflow ,whenever we try to initiate the workflow for the page activation its is throwing an error could not start the flow and we have verified the permission in etc workflow (read,create,edit,modify are set) and also we are able to see workflow running in the Instance in workflow console But couldn't find anything in workflow inbox for completing the workflow.

Few thoughts.

Is this an OOTB workflow or a custom one?

Any log traces that can be attached here?

Are the workflows going stale after some time?

This is the custom workflow.

Nothing get traced in error log

No workflow is not going to stale instead,we can see status as Running in the Instance in workflow console
