Combine/merge duplicate usernames

There are occasions when a username for one person is created twice. Proof approvals are then sent to both usernames and the user struggles to go back and forth between the accounts to approve the proof.

Duplicate accounts are created when the user has a name change but the person sending the proof approval is not aware of the new name so they will enter the old email address, creating an external user for the user's old email address.
