Capture and report on proof and document rejection reasons

We'd like our sign-off team to be able to pick from a drop-down list of reasons as to why they rejected a document or a proof. If we can report on this, then we can understand the most common reasons for rejections and amends, and give colleagues the support and training they need to stop getting rejected for the same things over and over.
